CAHP Fact Sheet: Enhanced Care Management and Community Supports: Making a Difference for California’s Most Vulnerable Communities

Enhanced Care Management (ECM) is a critical benefit under California’s Medi-Cal Managed Care program where Medi-Cal plans and providers provide comprehensive health care and treatment to some of the most vulnerable populations in the state. Under ECM, high risk Medi-Cal members receive enhanced and coordinated care so that all their health care needs are identified, addressed, and coordinated by a single case manager. Some of the populations helped by ECM include the unhoused, those with mental health and substance use disorders, and those at risk of being hospitalized. Medi-Cal Managed Care health plans play a vital role in implementing the ECM program and help ensure it is a success.
